Responsibilities:

1) Provides direct patient care to a select group of patients and advocates on their behalf
2) Assesses patient’s physical and psychosocial condition throughout the continuum of care.
3) Develops and communicate patient plan of care reflecting patient issues and proposed interventions to achieve desired patient outcome.
4) Collaborates with patient, family and healthcare team in implementing plan of care
5) Provides education as indicated after assessing the learning needs of the patient and family
6) Communicates effectively with all members of the health care team and related departments to provide accurate information about the care of the patients.
7) Formulates and maintains effective working relationships with peer, managers and other team members.
8) Displays a positive attitude and works to achieve an environment of respect, accountability, teamwork, integrity, and compassion. Does not support or take part in disruptive or negative behavior.
9) Respects and protects the patient’s rights to confidentiality and privacy and discloses information only for professional purposes which are in the patient’s best interests with full consideration of their legal rights.
10) Practices the 5 rights to patient safety when administering medications, blood or blood products and treatments. (right patient, right medication, right route, right dose, right time)
11) Responds to crisis in a calm, efficient manner, directing others concisely.
12) Evaluates patient outcomes, effectiveness of plan of care and readiness for discharge and/or continuing care. Revise plan of care as needed and appropriately refer for additional services where indicated.
13) Documents pertinent information, observations and all aspects of patient care clearly, concisely and in a timely manner.
14) Maintains equipment and utilizes supplies in a safe, cost effective manner
15) Participates in and supports performance improvement and risk management activities.
16) Participates in clinical research activities and demonstrates evidence base practice.
17) Participates in ongoing regulatory readiness compliance.
18) Supports hospital-wide and unit specific initiatives (e.g. Planetree, Magnet, Stroke Certification, Joint Certification, etc.)
19) Participates in the development and implementation of goals, objectives, policies and procedures and standards of care, which contribute to the achievement of the organization’s strategic plan
20) Completes required continuous training and education, including department specific requirements
21) Takes responsibility for own professional development.
22) Stamford Hospital encourages nurses to actively seek ways to enhance professional knowledge and competence through participation in professional nursing organizations
23) Complies with hospital attendance policy and department staffing policy and rotations.
24) Adheres to hospital and department specific dress code policy.
25) Complies with hospital and departmental policies and procedures and adheres to external agency requirements
26) Performs other related duties as assigned or requested in order to maintain a high level of service.
27) Will be cross trained to medical surgical floors for the ability to float as needed on a rotating basis when census permits.

Required Experience:

1) Maintains Basic Cardiac Life Support (BCLS) certification
2) Attends multidisciplinary discharge rounds
3) Obtain competency for CMS reportable quality indicators related to activities of daily living (IRF-PAI data)

Education/Certification Requirements:

1) Graduate of an accredited School of Nursing.
2) Valid license as Registered Nurse in Connecticut.
3) BSN strongly preferred.
4) BLS certification required.
5) Maintain minimum of 6 contact hours annually.

Manual Skills Requirement:

Significant portions of daily assignments involve application of manual skills requiring motor coordination in combination with finger dexterity (eg typing, equipment operation, administration of nursing care).

Physical Effort:

Mobility is necessary to provide patient care. Physical effort is required to assist patients with transfer and ambulation, which may involve lifting, pushing and pulling patients. Hearing within normal range is necessary in order to properly conduct assessments. Must be able to endure prolonged periods of standing and walking.

Physical Environment:

Regular exposure to blood/body fluids, chemicals and equipment used in the treatment of patients. Significant portions of daily assignments involve occupational risks such as cuts, needle puncture, exposure to toxic chemicals, injuries from falls or back injury sustained while assisting with moving, lifting or positioning patients, equipment or materials. May be required to manage the care of patients during altered states of consciousness and to perform physically demanding patient care.

Performs other related duties as assigned or requested in order to maintain a high level of service. Completes required continuous
training and education, including department specific requirements. Demonstrates professional work behavior by following Service
Standards and Success factors. Complies with departmental organizational policies and procedures and adheres to external agency
requirements.
