The Patient Liaison plays an integral role in delivering a highly personalized, coordinated, and exceptional patient experience at PALM Health. Working closely with an assigned integrative medicine physician, this individual serves as both a trusted partner to the provider and a primary point of contact for patients, ensuring seamless communication, thoughtful follow-through, and continuity of care.

The Patient Liaison is responsible for keeping the physician organized, informed, and prepared for each appointment while helping patients understand and navigate their individualized care plans. This includes reviewing physician documentation, communicating recommendations, coordinating services, answering questions within the scope of the role, and facilitating communication between patients, providers, and other members of the PALM Health team.

The ideal candidate is an experienced, highly organized healthcare professional with excellent judgment, strong communication skills, and the ability to build meaningful relationships with patients. This position requires a sophisticated understanding of healthcare delivery, a proactive approach to problem-solving, and a genuine interest in integrative medicine, preventive health, and whole-person wellness.

Requirements

- Bachelor's degree in social work, healthcare administration, health sciences, psychology, nursing, or a related field; equivalent relevant professional experience may be considered.
- Three or more years of relevant professional experience in healthcare, patient services, or clinical coordination.
- Experience working directly with physicians in a hospital, outpatient clinic, specialty practice, or concierge medicine environment.
- Previous professional experience in a healthcare setting, preferably in patient care coordination, medical social work, patient navigation, clinical practice support, or a comparable role.
- Strong understanding of healthcare terminology, medical documentation, and the coordination of patient care.
- Ability to interpret and accurately communicate documented physician recommendations, recognizing the boundaries between patient education and independent clinical advice.
- Exceptional organizational skills, attention to detail, and the ability to manage competing priorities in a dynamic clinical environment.
