As a member of the Addiction Consult Service, this position provides non-clinical support to individuals who present in an ED and/or Inpatient at Beverly and Addison Gilbert Hospitals with substance use-related needs. The Recovery Support Navigator serves as a recovery guide and resource navigator, helping individuals identify their treatment and recovery goals, understand available pathways to care, and overcome barriers to accessing services. This position works closely with the Addiction Consult Service Recovery Coach, healthcare providers, treatment centers, and community-based organizations to facilitate timely connections to detoxification, treatment, recovery, harm reduction, and other supportive services. As a member of the Addiction Consult Service, this position frequently engages individuals who have complex social and substance use-related needs, including those who are high utilizers of the ED. The Recovery Support Navigator contributes to care planning, facilitates referrals to community resources, follows up on referrals, and promotes continuity of care across the healthcare and community systems.
Job Description:
Essential Role Responsibilities
- Serve as a subject matter resource regarding the local addiction treatment and recovery support system, including pathways to detoxification, residential treatment, outpatient treatment, medication for opioid use disorder, recovery supports, harm reduction services, and community resources.
- Engage individuals in the ED setting who are seeking detoxification or other substance use treatment and provide education regarding available treatment and recovery options.
- Develop and maintain collaborative relationships with local detoxification and treatment centers to streamline referral, communication, and admission workflows.
- Identify available detoxification and treatment beds based on the individual's needs, preferences, and eligibility.
- Contact treatment centers with patients, when appropriate, to explore availability and facilitate admissions.
- Coordinate with the patient, treatment center, Recovery Coach, and healthcare team to obtain and submit required documentation for treatment referrals and admissions, including clinical and administrative information as appropriate.
- Track referrals and follow up with treatment centers and patients to help reduce barriers and support successful transitions to care.
- Provide follow-up telephone outreach to patients after ED or inpatient discharge to assess engagement with treatment, recovery supports, and other identified needs.
- During post-discharge outreach, identify individuals who may benefit from continued engagement with the Recovery Coach, peer recovery support, or other community-based recovery services and facilitate connection to those resources.
- Follow up with individuals who are discharged from the ED with Suboxone kits or harm reduction supplies to determine whether they have connected with an appropriate provider or service.
- Provide linkage to peer recovery networks, natural supports, mutual aid groups, community recovery organizations, and other resources when appropriate.
- Collaborate with the Addiction Consult Service Recovery Coach to ensure continuity of support and avoid duplication of services.
- Use effective time management and prioritization skills to respond to the needs of patients in the ED, admitted patients, and individuals requiring community-based follow-up.
- Document patient interactions, referrals, outreach efforts, and care coordination activities in Epic and complete other administrative documentation as required by the organization and supervisor.
- Advocate for individuals and support their self-determination while respecting individual choices, goals, culture, and readiness for change.
- Maintain appropriate professional boundaries and confidentiality while utilizing personal recovery experience.
- Collaborate, communicate, and advocate effectively with patients, families, healthcare providers, treatment facilities, community organizations, and other cross-functional teams.
- Complete required organizational and role-specific training and maintain competency in relevant addiction treatment, recovery, harm reduction, and community resource systems.
Qualifications
• High School diploma
• Training or relevant work or lived experience in the areas of mental health and/or addiction/recovery preferred.
Preferred Qualifications
• Bachelor's Degree in psychology, social work or related human services field is strongly preferred. May be able to accept candidates who are in their last semester of Bachelor's degree program under waiver.
• Experience delivering community based or hospital based services is a plus. Certificates, Licenses, Registrations, LADC/CADC preferred.
Pay Range:
$23.00 - $28.00
The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
