Collaborative Care (LICSW or LMHC)

Position Summary
The Collaborative Care (LICSW or LMHC) clinician supports integrated behavioral health services within
MFM Health’s primary care setting using the Collaborative Care Model (CoCM). This role engages and
assesses patients, coordinates individualized care with primary care providers and behavioral health
specialists, monitors treatment progress, responds to behavioral health needs, and maintains accurate
documentation to support effective, patient-centered care. This is a full-time in-office position servicing the Beverly and Danvers locations.

Core Responsibilities 
Patient Engagement & Assessment
- Engage patients in behavioral health care through education, support, and connection to appropriate resources.
- Conduct initial mental health assessments and screenings, including use of standardized assessment tools and evaluation of co-occurring conditions.
Care Coordination & Treatment Planning
- Coordinate communication, appointments, referrals, and follow-up among primary care providers, psychiatrists, therapists, social workers, and other care team members.
- Collaborate with patients and the care team to develop individualized, evidence-based treatment plans reflecting clinical findings and patient preferences.
- Monitor patient progress, track outcomes, follow up on treatment adherence and barriers to care, and coordinate treatment-plan adjustments with the care team.
Clinical Collaboration & Crisis Support
- Collaborate with multidisciplinary clinical teams and support primary care providers in addressing patients’ behavioral health needs. Provide crisis intervention support and coordinate emergency services or appropriate referrals when indicated.
Documentation, Education & Quality
- Maintain accurate and timely electronic health record documentation of assessments, patient interactions, treatment plans, follow-up, and outcomes.
- Support program reporting, performance monitoring, and quality improvement initiatives for the Collaborative Care Model.
- Provide education to healthcare colleagues regarding collaborative care practices and integration of behavioral health into primary care.
Required Qualifications
- Master’s degree in Social Work, Therapy, Counseling, or Psychology is required.
- Active Massachusetts licensure as a Licensed Independent Clinical Social Worker (LICSW) or Licensed Mental Health Counselor (LMHC) is required.
- Experience delivering behavioral health services in outpatient, community, hospital, or comparable clinical settings.
- Experience with care coordination, case management, or interdisciplinary clinical collaboration.
- Knowledge of the Collaborative Care Model (CoCM) and evidence-based behavioral health practices in primary care.
- Ability to conduct mental health assessments, communicate effectively with patients and clinicians, and document care accurately in electronic health records.
- Strong organizational skills, sound clinical judgment, and ability to respond appropriately to behavioral health crises.
Physical Requirements

- Ability to work at a computer and document clinical encounters for extended periods.
- Ability to communicate effectively with patients and colleagues in person, by telephone, and through electronic systems.
- Ability to move within clinical and office areas and perform duties at MFM Health practice locations.
- Ability to operate standard office and clinical documentation equipment.

Pay Range: $75,000–$90,000; commensurate with experience
