Our goal is to build positive relationships with the community to increase the access to appropriate mental health services for children and families who need them to improve their quality of life and ability to function more successfully in the community. Computer documentation of all services in a timely manner is required.
Efficiently schedule initial intake assessment appointments to facilitate the appropriate provision of medically necessary services to children with Serious Emotional Disturbance. Providing education to the community and partner agencies regarding services provided by COMCARE, who would be appropriate for the services, and how to access the services. Engage with community members to further COMCARE’s goals of increasing interest and access in COMCARE’s service array. Use expertise to mentor peers, guide internal customers through processes and protocols related to intake and assessment, foster care systems, school-based programming, and as well as connect to community partners.
Facilitate Access to Services
- Communicate with multiple community agencies to gain needed information to complete intake scheduling
- Complete thorough documentation in the medical record to clearly document relevant information to ensure quality care
- Ensure scheduled intakes have all required documentation and information
- Collect AIMS Data on each intake scheduled, as needed, and monitor and report compliance with access standards
- Assist in reviewing and facilitation of transferring the SED Waiver amongst CMHC
- Process electronically submitted documentation from external partners in a timely manner
- Provide transportation to support client attendance in medical appointments and other therapeutic goal related activities to include development of community resources, employment or education access, social or consumer-run activities and related supports
Community Education
- Stay abreast of available community-based education and outreach opportunities
- Develop and implement community education programs to increase awareness of agency services and how to access
- Represent COMCARE at community development meetings with a focus of improving service provision from a mental health perspective
Coordination of Care
- Conduct risk assessments as needed to address safety of patients accessing services.
- Collaborate with internal and external customers to facilitate communication among treatment providers to optimize patient outcomes
- Establish and maintain relationships with external providers, community organizations, to ensure available resources are provided to patients
- Coordinate referrals and transitions of care between other CMHC’s and COMCARE
Minimum Qualifications: Bachelor’s degree or be equivalently qualified by work experience or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education. Per Sedgwick County policy, this is a driving level position that requires a valid US driver’s license without restrictions and current proof of automobile insurance. Must be able to safely operate a motor vehicle and meet driver qualification standards as set forth in Sedgwick County’s Fleet Vehicle Operation and Usage policy. Must have access to personal passenger vehicle to complete the driving duties of the position. Meet the specifications as outlined in the CMHC/CCBHC licensing standards and pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens. Must complete orientations provided by Sedgwick County and COMCARE.
Preferred Qualifications: 1 year of experience in a Community Mental Health Center.
