In this role, you will provide high-quality nursing care to patients in a home care setting, ensuring their comfort and well-being through professional and attentive service.
Responsibilities
- Provide direct nursing care to patients in their homes, including administering medications and treatments as prescribed.
- Monitor patient health and document observations accurately.
- Collaborate with healthcare team members to develop and implement individualized care plans.
- Educate patients and their families on health maintenance and disease prevention.
- Maintain compliance with all regulatory and company standards for patient care.
Requirements
- Valid Licensed Practical Nurse (LPN) license in the state of Michigan.
- Previous experience in home health care or a similar nursing role preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Compassionate, patient-focused approach to care.
