Job Description

This position works with assigned patient populations and quality reports to identify open care gaps, contact patients when outreach is needed, retrieve supporting records from approved sources, document outreach and findings in Epic, and complete applicable care gap workflows when documentation requirements are met.

The position serves as a non-clinical member of the ambulatory care team and works collaboratively with providers, clinical staff, practice leadership, care coordinators, and quality teams to support organizational quality goals, improve documentation accuracy, and promote timely completion of preventive and chronic care services.

Minimum Qualifications

- High school diploma or equivalent required.
- Previous experience in healthcare administration, medical records, ambulatory operations, patient outreach, quality, or a related healthcare environment preferred.
- Experience using electronic medical records preferred.
- Basic knowledge of medical terminology preferred.
- Strong interpersonal and telephone communication skills.
- Strong computer, documentation, and data-entry skills.
- Ability to review high volumes of information with accuracy and attention to detail.
- Ability to maintain confidentiality and appropriately handle protected health information.
- Ability to work independently while recognizing when escalation is required.

Preferred Qualifications

- Associate or bachelor's degree in healthcare administration, health information management, public health, health sciences, or related field.
- Previous Epic experience.
- Experience with patient outreach, medical record review, chart abstraction, quality improvement, or care gap management.
- Familiarity with preventive care, chronic disease, and ambulatory quality measures.

Temporary Position Statemen t

This position is initially established as a temporary role to support focused ambulatory quality improvement and care gap initiatives. Based on demonstrated performance, organizational need, funding, and position availability, the role may be considered for transition to a permanent ambulatory quality, population health, or care management support position.

Additional Job Description
Bachelor's degree from an accredited college/university and one year of work experience in a Medical Office, Call Center, and/or customer service business environment; or a high school diploma or equivalent (GED) and three years of experience in a Medical Office, Call Center, and/or customer service business environment and a minimum of one-year satisfactory work experience in MUHA scheduling is preferred. The position requires strategic planning and analytic skills, ability to manage multiple projects, problem solving and service recovery skills; ability to work in a stressful, fast paced environment required. Must be able to work independently with attention to detail, strong written and verbal communication skills, and discretion with managing confidential patient and market information. Extensive experience with EPIC Cadence platform preferred. Proficiency with Microsoft Office Suite Products especially Excel, PowerPoint, and Word preferred.
