Pharmacists ensure safe, effective, timely, patient-centered and evidence-based care. The role requires interpreting and evaluating medication orders and providing medication expertise within the interdisciplinary team. All members of the Pharmacy team are responsible for developing and promoting collaborative and collegial relationships within the department and with nurses, physicians and interdepartmental personnel as they relate to the pharmaceutical care of the patients.

Job Description:

ESSENTIAL DUTIES & RESPONSIBILITIES (including but not limited to):

· Practices in accordance with the state and federal laws, rules, and regulations, including but not limited to Massachusetts Board of Registration in Pharmacy, Department of Public Health, the Food and Drug Administration, and the Drug Enforcement Administration. Maintains certification and credentials in accordance with the Board of Registration and adheres to institutional policies and procedures.

· Reviews and verifies medication orders with consideration, including but not limited to the following patient specific parameters: diagnosis, weight/body surface area, age, allergies, organ function, laboratory values, medication duplication, drug interactions. Clarifies orders deemed to be inappropriate, incomplete, or otherwise unclear with the prescriber in a timely manner.

· Documents and communicates pertinent information and discussion with the healthcare team and pharmacy staff to ensure continuity of care.

· Checks and verifies sterile and non-sterile medications.

· Provides drug information to patients, nurses, physicians, trainees and other employees.

· Ensures all legal and professional requirements are adhered to for controlled substances.

· Supervises, mentors and assists in training pharmacy technicians, interns and new pharmacists.

· Promotes a culture of patient safety by submitting medication related events and near misses.

· Completes daily pharmacy surveillance tool monitoring, including follow-up and documentation, as well as other daily patient care initiatives and relevant reports.

· Supports departmental workgroups to improve process and quality outcomes, Supports the development and revision of departmental clinical guidelines.

· Other duties as assigned.

· Pharmacists are responsible for their own professional development including meeting all requirements for continuing and education, keeping abreast of current professional literature, both from a clinical and medication safety perspective.

QUALIFICATIONS:

· Current MA Pharmacy License required

· BPS preferred

EDUCATION:

· Bachelor’s degree in pharmacy required

· Doctorate degree in Pharmacy preferred

· PGY-1 Residency or 1-3 years relevant hospital experience required

MINIMUM EXPERIENCE:

· 1-3 year(s) of work-related experience

Physical, Sensory, and Environmental Requirements

- Primarily sedentary work requiring prolonged sitting, frequent keyboarding, typing, writing, and computer use.
- Occasional standing and walking within the work area.
- Frequent reaching across a desk and handling, fingering, and keying of information technology equipment and office materials.
- Occasional grasping and close visual work, including reading documents, reviewing electronic information, and performing detailed computer-based tasks.
- May occasionally lift, carry, push, pull, or reposition lightweight materials or equipment, generally up to 20 pounds, with infrequent handling of weights up to 50 pounds.
- Requires visual acuity for close work, including viewing computer screens, reviewing reports, and completing documentation.
- Work is typically performed in a standard office or healthcare administrative environment with minimal exposure to adverse environmental conditions.
- Frequent independent work with occasional interaction with customers, end users, staff, and stakeholders.
- No regular exposure to hazardous materials, infectious diseases, bloodborne pathogens, bodily fluids, or other significant workplace hazards.

Pay Range:
$58.00 - $78.06

The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.  Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
