Occupational Therapist (OT) – Facility-Based | Rochester, NY | Full-Time & Part-Time
Stern at Home Therapy is seeking a Full-Time or Part-Time Occupational Therapist (OT) to provide individualized, one-on-one therapy services across three established facilities in the Rochester, NY area .
This opportunity is ideal for an Occupational Therapist who prefers working with an established patient population in a structured facility environment rather than traveling between individual patient residences.
Our facility-based model offers consistent patient referrals, flexible scheduling, clinical autonomy, and dedicated administrative support , allowing therapists to focus on delivering high-quality patient care.
Therapists will provide services across three facilities, with limited travel between locations.
Responsibilities
- Perform comprehensive occupational therapy evaluations and develop individualized plans of care.
- Provide one-on-one occupational therapy treatments to a primarily geriatric patient population.
- Help patients improve independence with activities of daily living (ADLs).
- Address upper-extremity strength, range of motion, coordination, balance, cognition, and functional mobility.
- Provide therapeutic activities, exercises, and adaptive techniques to improve functional independence.
- Recommend adaptive equipment and assistive devices when appropriate.
- Educate patients, caregivers, and facility staff on safety, adaptive strategies, and therapeutic interventions.
- Monitor patient progress and modify treatment plans as clinically appropriate.
- Complete accurate and timely clinical documentation.
- Collaborate with physicians, nursing staff, caregivers, and interdisciplinary team members.
- Coordinate with Stern's administrative team regarding scheduling and patient care.
Compensation
Full-Time – Minimum 16 Patients
Full-Time W-2
- $45–$50 per visit
- Fixed per-visit compensation
- No tiered compensation structure
Full-Time 1099
- $55 per visit
- Fixed per-visit compensation
- Independent contractor opportunity
Part-Time – Minimum 10 Patients
Part-Time W-2
- $45–$50 per visit

Full-Time W-2 Benefits
- Health insurance and/or medical stipend
- 401(k) with annual employer match starting at 1.5% and increasing up to 7%
- 2 weeks of PTO after 6 months and a third week after 9 months
- 5 paid holidays , available immediately, with employees choosing which holidays to observe
- Student loan repayment assistance after the first year, starting at $125 per month
- $500 annual continuing education (CEU) stipend
- $250 monthly commuter stipend

Part-Time W-2 Benefits

- Prorated commuter stipend based on patient volume, up to $175 per month for clinicians seeing 10 patients
- Equipment provided
1099 independent contractors are not eligible for W-2 employee benefits.

Qualifications
- Active Occupational Therapist license in New York State required
- Graduate of an accredited Occupational Therapy program
- Reliable transportation required for travel between the three facilities
- Strong clinical assessment, treatment planning, and documentation skills
- Excellent communication and time-management abilities
- Ability to work independently and collaboratively within a facility environment
- Experience with geriatric patients, assisted living, or skilled nursing populations preferred but not required
