About this role
Northbay Integrative Primary Care believes that lasting health is created through sustainable lifestyle change, meaningful patient partnerships, and whole-person care. The Lifestyle Medicine Health Coach/Nutritionist serves as a trusted guide who empowers individuals to build healthier habits that improve quality of life, reduce chronic disease risk, and support long-term well-being. Success in this role is measured not only by knowledge shared, but by patients' ability to create lasting, meaningful changes that enhance their health and vitality.
At Northbay Health, the Lifestyle Medicine Health Coach Nutritionist is a key member of the Northbay Integrative Primary Care team, partnering with physicians and interdisciplinary providers to empower patients in achieving sustainable health through evidence-based lifestyle change. This role supports individuals in preventing, reversing, and managing chronic disease by guiding lasting behavior change using the principles of Lifestyle Medicine, nutrition science, and health coaching.
Unlike traditional medical nutrition therapy focused on acute illness, this position emphasizes whole-person wellness by addressing nutrition, physical activity, sleep, stress resilience, emotional well-being, social connection, and other lifestyle factors that influence long-term health outcomes. Through motivational interviewing, behavior change science, and personalized coaching, the Health Coach/Nutritionist helps patients identify barriers, build self-efficacy, and develop realistic, individualized action plans that promote lifelong healthy habits.
Working collaboratively within an integrative care model, this role supports patients through individual coaching, shared medical appointments, group education, culinary medicine programming, and coordination with providers including physicians, acupuncturists, behavioral health specialists, and movement professionals. The Health Coach/Nutritionist fosters an encouraging, patient-centered environment that inspires meaningful and sustainable lifestyle transformation.
