About this role
Adoption & Permanency Support Caseworker – Strengthening Families. Building Forever Connections.
More Than a Job. A Chance to Help Families Thrive!
Are you passionate about helping children find stability, supporting families through life's challenges, and making a meaningful difference in your community? Do you enjoy building relationships, connecting families with resources, and being someone they can count on when they need support?
Four Oaks is looking for a compassionate, dedicated, and resourceful Adoption & Permanency Support Caseworker to join our team!
In this role, you'll work alongside adoptive, pre-adoptive, kinship, and subsidized guardianship families, providing guidance, resources, and ongoing support throughout their permanency journey.
From preparing families for adoption to helping them navigate challenges after permanency has been established, you'll play an important role in strengthening family relationships, promoting stability, and helping children thrive in safe, supportive homes.
What You'll Do
Support Families. Strengthen Connections. Create Lasting Stability.
- Make a Lasting Impact: Provide individualized support to pre-adoptive, post-adoptive, kinship, and subsidized guardianship families to help maintain stability and prevent placement disruptions.
- Build Meaningful Relationships: Establish trusting relationships with children and caregivers, offering guidance, encouragement, and support throughout their permanency journey.
- Create a Plan for Success: Partner with families to develop individualized support plans that identify needs, available resources, and strategies for navigating challenges or crises.
- Be There When It Matters Most: Provide in-home crisis response when families need immediate assistance, helping maintain safety and prevent disruptions.
- Connect Families with Support: Develop and maintain adoption support groups in collaboration with Four Oaks team members and community partners.
- Work Together for Better Outcomes: Collaborate with RRTS Caseworkers, HHS, Juvenile Court Services, and other stakeholders to ensure families receive coordinated services and support.
- Support Smooth Transitions: Participate in in-person transition meetings with RRTS Caseworkers to help families feel informed, connected, and supported.
- Share Your Knowledge: Assist with adoption-related training and educational opportunities for families, colleagues, and community partners.
- Keep Things Organized: Maintain timely and accurate documentation of family contacts, support services, and required information using CareMatch and established procedures.
- Support Permanency Services: Assist with initial home studies and additional RRTS responsibilities as needed.
- Stay Connected: Travel to family homes, stakeholder meetings, trainings, and support group activities to provide services where they are needed.
- Grow Professionally: Participate in supervision, team meetings, and ongoing training opportunities.
Why Your Work Matters
Permanency Is More Than a Placement. It's a Lifelong Connection.
Adoption and guardianship can bring incredible opportunities for children and families, but the journey doesn't always end when permanency is achieved.
Families may continue to face challenges, need additional resources, or simply benefit from having someone who understands their unique experiences.
As an Adoption & Permanency Support Caseworker, you'll help:
- Strengthen family relationships and promote long-term stability.
- Support children and caregivers before and after adoption or guardianship.
- Reduce the risk of placement disruptions.
- Connect families with resources, education, and supportive communities.
- Help families navigate difficult situations with confidence and support.
- Create positive, lasting outcomes for children and families.
Your work helps ensure families don't have to navigate their permanency journey alone.
