Department: York Case Management Operations
Schedule
Part Time, Day Shift (48 hours biweekly)
Hours: Every weekend position (Saturday and Sunday 7:00Am-7:00Pm) / 50 out of 52 weekends a year (Some Holidays)
General Summary
Plans, organizes and implements social work for patients and families. Works in a team type assignment or Care Continuation Team (CCT) model (Case Management Nurse and Social Worker teams are assigned to physician practices) to address patient care transition needs.
Qualifications
Minimum Education:
- Bachelors Degree Must be degreed in social work or social welfare per PA Act 68 of 2008. Required
Work Experience:
- Less than 1 year Related experience Required
Licenses:
- Basic Life Support Upon Hire Required
- Licensed Social Worker Upon Hire Preferred or
- Licensed Clinical Social Worker Upon Hire Preferred
Knowledge, Skills, and Abilities:
- Excellent interpersonal/communications skills.
- Basic computer skills.
- Able to maintain confidentiality.
- Able to act in a professional manner.
- Maintains job competency.
