About this role
Job Description Summary
The Nurse Practitioner (NP) or Physician Assistant (PA) plays a vital role in delivering comprehensive, patient-centered primary care within Millennium Physician Group's value-based care model. This provider is empowered to practice at the top of their license while building meaningful relationships with patients and driving improved health outcomes. The NP/PA is responsible for evaluating, diagnosing, and managing acute and chronic medical conditions, providing preventive care services, developing individualized treatment plans, interpreting diagnostic studies, prescribing medications, and delivering patient education that supports long-term wellness and disease management.
Working collaboratively with physicians and an interdisciplinary care team, the NP/PA serves as a trusted clinical partner focused on providing high-quality, coordinated care across the continuum. This role offers a high degree of clinical autonomy, allowing providers to independently manage routine and moderately complex patient needs while consulting with collaborating physicians as appropriate. In addition to direct patient care, the NP/PA contributes to population health initiatives, quality improvement efforts, care gap closure, HCC documentation, and value-based care performance measures designed to enhance patient outcomes and reduce unnecessary healthcare utilization. The ideal candidate is a compassionate, team-oriented clinician who is committed to clinical excellence, patient advocacy, and delivering exceptional primary care experiences.
