- Develop risk management plans and provide chronic care management and palliative care services within scope of practice.
- Interpret laboratory studies, imaging results, and diagnostic findings to guide treatment decisions.
- Prescribe medications and order home health services, therapies, and durable medical equipment (DME) as appropriate.
- Identify and address quality measures, care gaps, and HCC documentation opportunities to support value-based care initiatives.
- Provide interventions that help prevent unnecessary emergency department visits and hospital admissions.
- Educate patients and caregivers on disease management, medication adherence, symptom management, preventive care, and advanced care planning.
- Coordinate care across primary care providers, specialists, caregivers, home health agencies, skilled nursing facilities, and community resources.
- Identify patients who would benefit from skilled nursing, therapy services, behavioral health support, social services, or other community-based resources.
- Ensure accurate, timely, and compliant documentation and billing within the electronic medical record.
- Collaborate with assigned Medical Assistants and practice leadership to support workflow efficiency, staff development, and quality improvement initiatives.
- Consult with supervising physicians regarding complex diagnoses, treatment plans, and patient management decisions as needed.

Qualifications

- Active Florida Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) license in good standing.
- National Board Certification as an APRN or PA.
- Current DEA registration or eligibility to obtain.
- Experience in Primary Care, Family Medicine, Internal Medicine, Geriatrics, or a related clinical specialty preferred.
- Strong clinical assessment, diagnostic reasoning, and chronic disease management skills.
- Knowledge of value-based care, population health management, quality measures, and HCC documentation.
- Experience working with electronic health records and clinical technology platforms.
