Responsibilities

- Provide high-quality nursing care in a home health setting according to patient care plans.

- Monitor patients health status and progress, reporting any changes promptly.

- Collaborate with healthcare professionals to ensure comprehensive patient care.

- Administer medications, treatments, and therapies as prescribed.

- Educate patients and their families about care techniques and health management.

- Maintain accurate and detailed patient records in compliance with company policies and regulatory requirements.

Requirements

- Licensed Practical Nurse (LPN) certification in the state of Virginia.

- Valid LPN license without restrictions or disciplinary actions.

- Strong communication and interpersonal skills.

- Ability to work independently and follow care plans effectively.

- Prior experience in home health care or a related nursing field preferred.

- Reliable transportation to patients homes in Glen Allen, VA.
