Phoenix Home Care & Hospice is looking for a Care Coordinator & Scheduler who can think quickly, communicate clearly, and keep the day moving.
Think of yourself as the person behind the controls — coordinating schedules, matching nurses with clients, and making adjustments when the unexpected happens.
YOUR OBJECTIVES
- Match nurses with clients and build dependable schedules
- Manage and update scheduling systems
- Communicate with nurses, caregivers, clients, and families
- Solve scheduling problems and last-minute changes
- Support your team and jump in when needed

Experience in customer service, call centers, scheduling, staffing, or office coordination is preferred.
