We are seeking a dedicated and compassionate Transplant Coordinator to join our Kidney Transplant Programme. This role plays a vital part in supporting kidney transplant recipients throughout their long‑term recovery and ongoing care.
Key Responsibilities
- Conduct patient interviews and perform preliminary health assessments
- Coordinate investigations, clinic appointments, and follow‑up care
- Monitor medication adherence and provide patient counselling
- Educate patients and families on self‑care and post‑transplant health
- Maintain accurate patient records and support data reporting
- Participate in quality improvement initiatives and research activities
Requirements
- Degree in Nursing, Psychology, or a Health‑related field
- Experience in healthcare or other people‑centric roles
- Strong communication, teamwork, and patient‑education skills
- Willingness to perform on-call duties on a rotational basis
