Principal Responsibilities

- Conduct phone screenings to collect biodata and gather social information for medical social workers. Contact patients for appointment reminders and rescheduling.

- Accompany medical social workers on field visits or conduct them independently.

- Timely completion of case recordings.

- Liaise with external agencies .

- Transcribe applications onto online referral systems.

- Monitor and follow up on referral statuses.

- Handle external correspondences.

- Attend to Medical Social Services enquiry emails.

- Maintain work statistics and perform clerical duties.

Preferred Qualifications!

- Minimum 1 year of relevant experience in social services or healthcare is preferred

- GCE ‘A’ or ‘O’ Level with passes in English and Mathematics

- Certificate/Diploma in community and social services or equivalent preferred
