Transition Outreach Specialist
We're seeking a compassionate and motivated Transition Outreach Specialist to help students with developmental disabilities successfully transition from school into adult services and community programs.
Key Responsibilities
- Connect with students, families, and schools to support transition planning.
- Attend IEP meetings and provide guidance on available resources and services.
- Assist families with benefits, funding opportunities, and community supports.
- Build strong relationships with school districts and community partners.
- Advocate for individuals and help them achieve their goals for adulthood.
Qualifications
- Bachelor's degree in Human Services, Social Work, Psychology, or a related field.
- 3+ years of experience working with individuals with developmental disabilities.
- Valid driver's license and reliable transportation.
- Bilingual English/Spanish preferred.
