ABOUT THE ROLE

Matter Health is seeking compassionate, dependable individuals to join our team as Care Facilitators . This seasonal, part-time, field-based position plays an important role in helping older adults access healthcare by connecting them with Matter Health providers through virtual visits.

- Follow a predetermined daily schedule and move efficiently between scheduled patient visits.
- Communicate scheduling delays, patient availability concerns, or technology issues promptly to the Market Operations Leader.
- Keep company-provided iPads and equipment charged, secure, and ready for use throughout the workday.
- Document visit-related issues, including missed appointments, declined consent, or technology challenges, according to established procedures.
- Maintain patient confidentiality and comply with HIPAA and Matter Health policies.
- Complete required onboarding and training before beginning patient visits.
- Demonstrate empathy, professionalism, and respect in every interaction with patients, providers, and community members.

QUALIFICATIONS

- Dependable, punctual, and able to follow a structured daily schedule.
- Ability to work independently while maintaining communication with the Market Operations Leader.
- Strong interpersonal skills and a compassionate, patient-centered approach when working with older adults.
- Good organizational skills and attention to detail.
- Ability to successfully complete a background check.

PREFERRED QUALIFICATIONS

- Experience working with older adults or in senior housing, affordable housing, home care, healthcare, hospitality, or customer service.

- Creating a welcoming, comfortable experience for older adults during their first virtual healthcare appointment.
- Ensuring patient consent is completed accurately before each visit begins.
- Connecting patients and providers successfully through the virtual visit platform.
- Maintaining accurate visit documentation and promptly communicating challenges or delays.
- Protecting patient privacy and maintaining professional boundaries.
- Serving as a dependable, compassionate representative of Matter Health within the communities we serve.
