- Hours: Full-time position
- Travel: Ability to spend approximately 20% of the day driving to/from patient locations
- Schedule: Monday-Friday, 8am-5pm *no on-call or weekends
The Role
Join our client's care team as a Home Health Physical Therapy Assistant, providing compassionate support to patients in their homes. Key Responsibilities
- Collaborate to provide management and delivery of patient care plans
- Support patients in Marion, IL and surrounding areas
- Meet critical deadlines for charting
- Self-manage time and schedule effectively
Qualifications
- Associate degree or higher
- 1+ years of experience in ANY of the following:
Home Health Care
- Home Health
- 1+ years of experience in ANY of the following:
Electronic Medical Record (EMR)
- Practice Partner EMR
- EHR Implementation
- Valid Regular Driver License (DL) certification/license in the state of Illinois (IL)
- Valid Physical Therapy Assistant certification/license in the state of Illinois (IL)
