The Role:

Join our diverse and dynamic Adults’ Health and Care Brokerage Team as a Senior Brokerage Officer and play a vital role in helping people leave hospital safely and receive the care and support they need.

As part of Brokerage and the wider Payments and Customer Financial Interactions Service, our Hospital Liaison function works closely with Acute and Community Hospitals, Adult Social Care teams, families, and care providers to secure appropriate long-term care arrangements for adults with eligible social care needs.

In your role you’ll combine complex case management with leadership responsibilities, supporting colleagues, resolving challenging sourcing issues and working collaboratively with a wide range of partners. Through your expertise and influence, you'll help ensure people receive the right care, in the right place, at the right time, while contributing to the effective operation and continuous improvement of the Brokerage Service.

What you’ll do:

- Manage a complex caseload, securing suitable Residential and Nursing Care placements that meet assessed needs and support timely hospital discharge.

- Provide guidance and support to Brokerage Officers, helping to resolve complex sourcing challenges and escalations.

- Build strong partnerships with hospitals, social care teams, care providers and other stakeholders to support timely discharges and service improvement.

- Represent the Brokerage Service at meetings, contributing to collaborative decision-making and problem-solving.

- Confidently lead sensitive and complex conversations with individuals, families, representatives and providers.

- Negotiate with providers and operational teams to secure high-quality, cost-effective long-term care solutions that support individual outcomes and deliver value for money.

- Use performance data and management information to identify trends, inform operational decisions, escalate emerging pressures, support service improvements and deliver value-for-money outcomes.

- Play a key role in brokering suitable care packages and placements, ensuring services offer value for money while maintaining quality and meeting individual requirements.

What we’re looking for:

- Experience working within adult social care, health, brokerage or a related service environment.

- Knowledge of Residential and Nursing Care services and the challenges associated with hospital discharge.

- Experience managing complex casework and coordinating services to achieve positive outcomes.

- The ability to communicate confidently and compassionately, build strong working relationships, and influence and negotiate effectively.

- Confidence in handling challenging situations and sensitive conversations with professionalism and empathy.

- Strong analytical and problem-solving skills, with the ability to make informed decisions and identify practical solutions.

- The ability to prioritise competing demands, support colleagues and thrive in a fast-paced environment.
