Responsibilities
- Provide skilled nursing care to patients in their homes, including medication administration, wound care, and monitoring vital signs.

- Document patient care activities accurately and maintain compliance with company and regulatory standards.
- Educate patients and their families on health maintenance and disease prevention.
- Respond promptly and effectively to changes in patient condition or emergencies.
- Maintain professional standards and confidentiality at all times.
Requirements
- Valid Licensed Practical Nurse (LPN) license in the state of Michigan.
- Experience in home health care or a similar nursing environment preferred.
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a multidisciplinary team.
- Reliable transportation to travel to patient homes.
