Responsibilities
- Provide direct patient care including administering medications, monitoring vital signs, and assisting with daily living activities.
- Collaborate with healthcare professionals to develop and implement individualized care plans.
- Document patient progress and maintain accurate medical records.
- Educate patients and families on healthcare management and disease prevention.
- Ensure compliance with healthcare regulations and company policies.
Requirements
- Valid LPN license in the state of Michigan.
- Previous experience in nursing, preferably in home care or a related setting.
- Strong communication and interpersonal skills.
