About the Role
We are seeking an experienced RN or LPN for a full-time, field-based role . This position requires daily travel to client homes to complete supervisory visits, assessments, and ongoing care management.
This is an active field position that requires frequent walking throughout the workday and the ability to safely lift, carry, and move items weighing at least 50 lbs.
You will be working independently in the field while collaborating closely with case managers, families, and our internal team.
Compensation & Benefits
- Salary: $90,000–$105,000 annually (based on experience)
- Mileage reimbursement provided
- Full-time, stable Monday–Friday schedule
- Flexible schedule options: 7:00 AM–3:00 PM or 8:00 AM–4:00 PM
- No weekends
Key Responsibilities
- Complete in-home supervisory visits for HHA/PCA caregivers
- Conduct client assessments and reassessments
- Ensure compliance with ASAP program requirements
- Communicate with ASAP case managers and families
- Monitor client conditions and report changes
- Maintain accurate and timely documentation
Requirements
- Active RN or LPN license in Massachusetts
- Minimum 1 year of home care experience preferred, but not required (new grads encouraged to apply)
- ASAP experience strongly preferred
- Must be comfortable working independently in the field
- Must be comfortable with frequent walking throughout the workday
- Must be able to safely lift, carry, and move at least 50 lbs
- Reliable transportation and willingness to travel daily
- Strong assessment and documentation skills
- Bilingual Spanish preferred, but not required

- Not remote or office-only
- Not a non-clinical/companion role
Schedule
- Full-Time
- Monday through Friday
- Flexible 7:00 AM–3:00 PM or 8:00 AM–4:00 PM
- No weekends
- Day shift with daily travel required
