Home Health Physical Therapist (PT)

Billet Health is looking for a compassionate, skilled, and dedicated Physical Therapist (PT) to join our growing Home Health team serving patients throughout the Phoenix Valley .

This per diem opportunity offers flexibility, autonomy, and the opportunity to provide one-on-one skilled care directly in patients’ homes —helping them regain mobility, improve function, and maintain independence.

Your Role

As a Home Health Physical Therapist, you will evaluate and treat patients in their homes while developing individualized plans of care based on each patient’s needs and physician orders.

Responsibilities

- Perform comprehensive patient evaluations and assessments

- Develop individualized treatment plans based on physician orders

- Provide skilled physical therapy services in patients’ homes

- Monitor patient progress and modify treatment plans as appropriate

- Educate patients and caregivers on exercises, mobility, fall prevention, and safety

- Collaborate with physicians and interdisciplinary care team members

- Maintain accurate, complete, and timely clinical documentation

- Promote patient independence and safe mobility in the home environment

Qualifications

- Graduate of an accredited Physical Therapy program

- Current Arizona Physical Therapist license

- Valid driver’s license and reliable transportation

- Current auto insurance

- CPR or BLS certification

- Current TB documentation

Skin test within one year, or

- Chest X-ray within five years

- Home Health experience preferred
