About the Role
Clarvida is seeking a High Fidelity Wraparound Care Coordinator to support youth and families with complex behavioral health needs through High Fidelity Wraparound (HFW) Services. In this role, you will facilitate a family-driven, strengths-based planning process that brings together family members, natural supports, community partners, and service providers to create individualized plans focused on achieving meaningful outcomes.
The High Fidelity Wraparound Care Coordinator serves as the facilitator of the Child and Family Team, ensuring that youth and families are empowered to lead the planning process while coordinating supports across home, school, community, healthcare, and behavioral health systems. HFW is designed for youth through age 20 and emphasizes family voice and choice, collaboration, and sustainable community-based solutions.
Responsibilities
- Facilitate the High Fidelity Wraparound process using family-driven, strengths-based, and team-based planning practices.
- Engage youth, caregivers, family members, and support systems to develop a shared family vision, mission, and plan of care.
- Coordinate and facilitate Child and Family Team meetings to support individualized service planning and goal achievement.
- Develop, maintain, and regularly review individualized plans of care and crisis and safety plans.
- Partner with Family Support Specialists, Youth Support Specialists, and other identified team members to promote successful outcomes.
- Coordinate services among behavioral health, medical, educational, child welfare, juvenile justice, developmental services, and community providers.
- Assist youth and families in identifying strengths, natural supports, and community resources that support long-term success.
- Monitor progress toward established goals and modify strategies collaboratively with the team as needed.
- Support successful transitions across home, school, treatment, and community environments.
- Complete assessments, service plans, progress notes, and other required documentation in compliance with agency, contractual, and state requirements.
- Participate in supervision, fidelity monitoring, professional development activities, and quality improvement initiatives associated with the High Fidelity Wraparound model.
- Provide services in homes, schools, and community settings throughout the assigned service area.
Requirements
Required Qualifications
- Bachelor's Degree in Social Work, Psychology, Human Services, Education, Counseling, Behavioral Health, or a related field
- Minimum of one (1) year of experience working with children, youth, and families with behavioral health, emotional, developmental, or complex social service needs
- Strong communication, facilitation, organizational, and documentation skills
- Ability to build collaborative relationships with youth, families, providers, schools, and community partners
- Ability to work independently while managing a community-based caseload
- Valid Driver's License, reliable transportation, and active automobile insurance
- Ability to accommodate occasional evening meetings based on family availability
Preferred Qualifications
- MHRT-C certification or eligibility
- Experience facilitating team-based planning meetings
- Experience working within children's behavioral health, child welfare, education, juvenile justice, developmental services, or community support systems
- Knowledge of High Fidelity Wraparound principles, Systems of Care, and family-driven planning practices
- Experience coordinating services across multiple agencies and systems
