POSITION TITLE: Care Coordination Program Supervisor

REPORTS TO (TITLE): Care Coordination Program Manager

Job Summary:

Under the direction of the Care Coordination Program Manager and with support from the Program Director of Special Populations Programs, the Program Supervisor oversees the daily operations of the Care Coordination program and the primary goals for this position include creating safe and efficient clinic and community-based workflows and data tracking systems, coordinating staff training and education, supervising staff schedules, making sure staff follow safety measures on in home visits and ensuring effective communication between program staff, clients/patients, community partners, management and grant funders.

Salary Range $70,304- $87,000 DOE

Specific Tasks/Duties Include :

- Assists in the management of day-to-day program operations to ensure patient, staff, and programmatic needs are met.

- Directly supervises and develops Care Coordination Dept. employees. Staff members include care coordinators, community health workers, and any other non-licensed personnel hired within the department.

- Must be familiar with the Collective Bargaining Agreement in the supervision of any union-represented staff.

- Monitors and manages staffs daily schedules to ensure productivity and staff safety.

- Communicates with care coordination management to ensure effective relay of information that relates to program operations.

- Orients and trains new staff on program and department operations and coordinates ongoing training and education for existing staff.

- Corrects problems with scheduling or workflows and makes recommendations for systems improvement.

- Guarantees program meets all grant requirements and expectations including documentation of program and clinical support tasks and data entry.

- Ensure that care plans are person-centered, and address clients' medical, behavioral, and social needs, emphasizing the integration of behavioral health and primary care services and are aligned with program goals.

- Facilitate regular case reviews with an interdisciplinary team to support comprehensive and coordinated care.

- Participates in quality improvement projects and other duties as assigned by manager.

- Acts as a liaison in community meetings with partners to support grant and program objectives.

- Oversees ordering of medical and office supplies for the department.

- Develops and maintains working relationships with community partners.

- Responsible for timely, accurate submission of timekeeping processes to payroll.

- Reviews incoming program referrals daily and ensures that all referrals are distributed to appropriate staff and programs in an efficient manner.

- Supports Program Manager with submission of accurate and complete documentation and timely reporting to funders or as needed for compliance.

- Participate in ongoing training and reviews.

- Other duties as assigned.

Qualifications

Minimum Qualifications:

- Minimum of 2 years working in a medical Care Coordination environment.

- Community Health Worker certificate, Medical Assistant Certificate, Certified Nursing Assistant Certificate, associate or bachelor’s degree in related field or relevant experience required.

- Minimum of one year of leadership experience or 2 years of related Care Coordination experience with current employment at SRCH preferred.

- Fluency in Spanish, spoken and written, preferred.

- Valid driver’s license with clean driving record and auto insurance.

Knowledge and Skills:

- Demonstrates proficiency with all tasks on the most current Care Coordinator job description.

- Strong verbal and written communication skills.

- Possess a deep understanding of Care Coordination fundamental and workflows.

- Demonstrates knowledge of compliance issues within the community clinic environment.

- Ability to prioritize projects, work relatively independently, manage multiple tasks, and meet deadlines.

- Ability to effectively present information, respond to questions, and generally communicate clearly and respectfully with participants, coworkers, other agencies and companies.

- Proficiency with Microsoft Office applications including Outlook, Word, Excel and PowerPoint.

- Good judgment, problem solving and decision-making skills.

While performing the duties of this job, this position is frequently required to do the following:

- Tolerate working outside in fluctuating weather conditions, including hot and cold temperatures and occasional wet weather.

- Travel to other office and community locations.

- Lift up to five (5) pounds.
