Affirmed Home Care is seeking a compassionate, dependable, and skilled Licensed Practical Nurse (LPN) to join our team and provide high-quality, one-on-one care to a client in Newark, New Jersey .

What You’ll Do
- Provide compassionate nursing care according to the client’s individualized care plan
- Administer medications and prescribed treatments
- Monitor the client’s condition and promptly communicate any changes
- Provide wound, catheter, and other skilled nursing care within your scope of practice
- Maintain accurate and timely clinical documentation
- Promote the client’s comfort, safety, dignity, and independence
- Communicate professionally with the client, family members, and care team
What We’re Looking For
- Active New Jersey LPN License
- 1+ year of nursing experience preferred
- Current CPR/BLS certification
- Strong clinical, communication, and organizational skills
- Reliable, compassionate, and professional
- Comfortable working independently in a home care setting
- Valid driver’s license and reliable transportation preferred
- EHR/documentation experience preferred
