As a Chemical Health Professional (LADC) , you will provide dual-diagnosis treatment services, counseling, and case management to adults and families participating in substance use disorder treatment and recovery services. From program entry through discharge, you will serve as a primary point of contact, helping participants build recovery goals, strengthen support systems, and access the resources they need to live healthier, more self-directed lives.

What You'll Do
- Provide individual counseling, case management, and treatment services for adults and families participating in substance use disorder treatment
- Develop treatment plans and support participants in identifying and achieving recovery goals
- Facilitate educational and process groups using evidence-based and participant-centered approaches
- Coordinate care with healthcare providers, community organizations, and other service partners
- Connect participants with housing, healthcare, transportation, recovery supports, and community resources
- Utilize motivational interviewing, trauma-informed care, and recovery-oriented practices throughout service delivery
- Promote participant health, wellness, and self-directed recovery
- Maintain accurate and timely documentation in electronic health record systems
- Collaborate with multidisciplinary teams to support participant success and continuity of care
- Support culturally responsive services and Avivo's commitment to reducing disparities and promoting dignity for all participant
Qualifications
What You Bring
- Bachelor's degree required; master's degree preferred
- Current Minnesota Licensed Alcohol and Drug Counselor (LADC) license in good standing
- Six months to one year of related experience
- Minimum of two years free from substance use problems
- Experience facilitating groups and engaging participants in treatment services
- Knowledge of substance use disorders, mental health, recovery principles, and community resources
- Experience working with or serving diverse populations
- Understanding of 245G regulations and treatment documentation requirements preferred
- Strong communication, assessment, and case management skills
- Ability to work independently while collaborating effectively within a multidisciplinary team
