POSITION SUMMARY:
This is a limited-term position funded through a grant agreement until (DATE) and contingent upon funding extension.
{If Hybrid, add schedule, ex.1 day onsite and 4 work from home days}.
ROLE & RESPONSIBILITIES:
- Conduct comprehensive assessments with immigrant families, asylum seekers, parolees, and visa holders to identify needs, overcome barriers, and connect them with appropriate community resources.
- Assist families in accessing and understanding public benefits programs, including Medicaid, All Kids, VTTC, and WIC.
- Orient participants to the healthcare system by helping schedule and attend appointments, arranging or providing transportation, and assisting with physician communication.
- Educate families on navigating their new surroundings in Chicagoland and research community resources, such as food pantries, housing support, ESL classes, healthcare, and legal services.
- Maintain timely, accurate, and confidential digital and physical client files, case notes, and internal database tracking systems.
- Actively participate in team meetings, program trainings, and supervision check-ins while applying trauma-informed practices and high confidentiality standards.
- Other duties as assigned.

- Valid drivers license, access to reliable vehicle and ability to pass MVR standards
PREFERRED QUALIFICATIONS:
- Bachelor’s degree in Social Work, Human Services, International Relations, or a related field (or equivalent combination of education and/or direct service experience/customer service experience).
- Personal or professional understanding of the immigrant experience and navigation of local social service systems.
- Proficiency or fluency in a second language spoken by local client communities (e.g., Spanish, Dari, Pashto, Ukrainian, Russian, Arabic or French) highly preferred.
- Knowledge of and experience working with traumatized populations, utilizing trauma-informed approaches and strengths-based practices.
- Highly self-motivated and adaptable, with a strong ability to work independently in a fast-paced, evolving environment.
- Strong mediation, critical thinking, and problem-solving skills, with a track record of effective advocacy on behalf of clients with service providers and government agencies.
- Excellent organizational skills with proven follow-through on assignments, complex paperwork, and project deadlines.
- Exceptional relational and intercultural communication skills, with an ability to build trust across diverse backgrounds.
