Position Summary : The Health Navigator will work within our Federally Qualified Health Centers (FQHCs) to support patients in accessing and navigating healthcare services with a focus on the Medication to Address Opioid Use Disorders (MOUD) program and closing essential gaps in care. This role is essential in improving health outcomes by providing education, resources, and guidance for underserved populations, including adults with Serious Mental Illness, uninsured, underinsured, and vulnerable community members. The Health Navigator collaborates with healthcare providers, case managers, and external community resources to promote patient-centered care and facilitate access to essential health services. The Health Navigator will provide BHT level behavioral health and skills interventions to facilitate overall wellness and health to patients served. This position reports to the Health Center Director or Clinical Site Manager.
Duties May Include:
- Provides outreach, engagement, care coordination, and patient navigation services within Mobile Health programs.
- Conducts outreach in shelters, schools, community centers, outreach locations, community events, and other community-based settings.
- Builds trusting relationships with patients and community members to improve healthcare engagement.
- Assists patients in accessing primary care, behavioral health, substance use treatment, specialty care, and community resources.
- Identifies potential barriers to quality care and opportunities for improvement and reports them to the Operational Supervisor and/or the Director of Nursing.
- Coordinates referrals and tracks completion of follow-up services.
- Ensures documentation of patient encounters in accordance with policies, procedures, and clinical protocols same date of service.
- Supports transitions of care following emergency department visits, hospitalizations, and crisis encounters.
- Conducts Social Determinants of Health screenings and assists patients with housing, transportation, food assistance, insurance, employment, and other support services.

Qualifications
- High School Diploma or GED required.
- Associate degree preferred.
- Minimum one year experience in care coordination, outreach, navigation, social services, healthcare, behavioral health, or related field.
- Knowledge of community resources and social determinants of health.
- BLS required.
- Must pass a TB Test.
- Knowledge, Skills, & Abilities: Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members.
- Computer Skills: Microsoft Word, Excel, PowerPoint, Outlook, electronic medical record, NextGen preferred.
- Must have valid Arizona drivers license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Healths driving policy.
- Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
- Preferred: Experience in mobile health, FQHC, public health, population health, or outreach programs.
- Preferred: Experience serving vulnerable, homeless, uninsured, or underserved populations.
- Preferred: Bilingual English/Spanish.
- Community Health Worker certification preferred.
- Ability to work independently in community-based settings and adapt to changing environments.
