Medical Record Clerk II

Job Description

The Medical Record Clerk II performs analysis of hospital inpatient, hospital outpatient surgery and Emergency Room records for deficiencies.  Assists physicians and other clinical staff with completing their patient records timely in accordance with Medical Staff Rules and Regulations.  Also, performs quality audits and clinical pertinence reviews to support Corporate Compliance, Patient Safety, Regulatory agencies (TJC, DOH, et al.)  The Record Analyst is responsible for monitoring and processing Epic work queues and reports.

Essential Functions

- Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
- Analyze inpatient, hospital outpatient surgery and ED records for deficiencies.  Assign deficiencies if required information is missing in the medical record according to the guidelines and schedules provided by each department.
- Professional outreach as needed to physicians to complete overdue deficiencies.
- Determines if escalation to manager is required on outstanding record responsibilities where physicians are non compliant.
- Monitors the status of final reports for procedures or surgeries to facilitate documentation needed for continued patient care and coding.
- Monitors Epic work queues as assigned including but not limited to:  History & Physicals, Operative Note, Verbal/co-signed orders and verification of scanned images that update deficiencies.
- Maintains monthly quality score and monthly productivity in accordance with metrics established for hospital inpatient, HOS and Observation accounts.
- Performs other duties as assigned.
