Licensed Practical Nurse (LPN) – Home Health
Full-Time | Boston Territory
Monday–Friday, 8 a.m.–5 p.m. | $38-$42/hour, based on experience

A great nursing visit can change someone’s entire day. It might mean recognizing a concern early, helping a caregiver feel more confident, or supporting a patient as they regain independence.
At Elara Caring , we’re seeking a Licensed Practical Nurse to bring compassionate, skilled care to patients throughout Boston . You’ll build meaningful relationships through one-on-one visits and work closely with an RN Case Manager and clinical team to support each patient’s care.

What You’ll Do
- Provide skilled nursing care according to each patient’s individualized care plan and physician orders.
- Monitor patient progress and promptly report changes in condition or safety concerns to the RN Case Manager.
- Teach patients and caregivers self-care techniques that support health, safety, and independence.
- Collaborate with the RN Case Manager as patient needs and care plans change.
- Document visits, nursing interventions, and medication information accurately and promptly.
- Communicate with physicians, families, and the clinical team to keep care coordinated.
- Help reduce avoidable hospitalizations through attentive care and early recognition of concerns.
What You’ll Bring
- An active Massachusetts LPN license
- Graduation from an accredited nursing program
- At least one year of clinical nursing experience
- A valid driver’s license, reliable transportation, and current auto insurance
- Ability to travel throughout the Boston territory
- Ability to sit, stand, bend, lift, and move intermittently, including lifting 50–100 lbs.
