Home Health Licensed Practical Nurse (LPN)

Full-Time | Monday–Friday, 8 a.m.–5 p.m.
$38–$42/hour, based on experience | No Weekends or On-Call

At Elara Caring , we’re seeking a Licensed Practical Nurse to deliver compassionate, skilled care in patients’ homes. You’ll build meaningful relationships through one-on-one visits while working closely with an RN Case Manager and a supportive clinical team.

What You’ll Do

- Provide skilled nursing care according to each patient’s individualized care plan and physician orders.
- Monitor patient progress and promptly report changes in condition or safety concerns to the RN Case Manager.
- Teach patients and caregivers techniques that support health, safety, and independence at home.
- Collaborate with the RN Case Manager as patient needs and care plans change.
- Document visits, nursing interventions, and medication information accurately and promptly.
- Communicate with physicians, patients, families, and the clinical team to keep care coordinated.
- Help reduce avoidable hospitalizations through attentive care and early recognition of concerns.

What You’ll Bring

- An active Massachusetts LPN license
- Graduation from an accredited nursing program
- At least one year of clinical nursing experience
- A valid driver’s license, reliable transportation, and current auto insurance
- Ability to travel between patient homes and assigned work locations
- Ability to sit, stand, bend, lift, and move intermittently, including lifting 50–100 lbs.
