Position Summary
The Case Manager delivers direct community-based suicide-prevention case management for the ESME –U.S. Department of Veterans Affairs Staff Sergeant Parker Gordon Fox Suicide Prevention Grant Program (ESME - SSG Fox SPGP). The position combines veteran outreach and engagement, eligibility and suicide-risk screening, individualized service planning, care coordination, supportive-service navigation, warm handoffs, participant follow-up, and accurate documentation.
Working as one of two (2) Case Managers, the position is directly responsible for helping ESME - SSG Fox SPGP achieve annual commitments of 125 participant enrollments and at least 175 eligibility/suicide-risk screenings. Each Case Manager is expected to maintain an active referral and enrollment pipeline, manage an assigned caseload and geographic coverage area, complete required VA screening and service-planning activities on time, and sustain participant engagement through service completion and exit.
The Case Manager is supervised by the Director of Military and Veteran Services and works closely with the Program Director and the Data & Quality Analyst. The position provides non-clinical suicide-prevention services within the approved SSG Fox SPGP scope and follows established escalation and emergency-response protocols when a participant presents urgent risk. The Case Manager does not substitute for emergency or clinical care and does not independently provide clinical treatment outside allowable grant parameters.
The Case Manager delivers services in community settings throughout assigned rural and underserved Maine counties and coordinates with VA facilities, Vet Centers, veteran-serving organizations, behavioral-health and crisis partners, housing and benefits providers, food-security and transportation resources, legal and financial-stability partners, and other community organizations. Reliable transportation and regular travel are required. Mileage reimbursement is provided for work-related travel from assigned work locations.
The Case Manager is accountable for participant-level execution: outreach, eligibility, screening, enrollment, service planning, referrals, supportive-service coordination, follow-up, required pre-/post-screenings, exit activities, and complete case documentation. The role also contributes outreach and partner intelligence to the Program Director so referral pipelines and geographic coverage can be adjusted to meet grant pacing. Travel Requirements: Frequent statewide travel – especially within Aroostook, Hancock, Penobscot, Piscataquis, Somerset, Waldo, Washington counties

Qualifications
Education/Certifications
- High school diploma or GED required; associate or bachelor’s degree in social work, human services, behavioral health, psychology, veteran services, public health, or a related field is strongly preferred.
- Minimum two (2) years of experience in veteran services, case management, social services, behavioral health, peer support, crisis-response systems, community outreach, benefits navigation, or a related field.
- Strong ability to engage veterans and other eligible participants experiencing multiple risk factors; build trust; use motivational and participant-centered approaches; and maintain professional boundaries, confidentiality, and accurate documentation.
- Ability to follow structured suicide-risk screening, safety, crisis-escalation, emergency-response, and warm-handoff protocols without functioning outside the scope of the position.
- Valid driver’s license, reliable transportation, automobile insurance, and ability to travel regularly throughout assigned service areas and statewide as needed.
Experience
- Knowledge of veteran and military culture; suicide-risk and protective factors; participant engagement; case management; care coordination; supportive-service navigation; crisis escalation; and VA/community referral systems.
- Ability to conduct structured interviews and required screenings, translate identified needs into an individualized suicide-prevention service plan, track referrals and participant progress, and close the loop on warm handoffs.
- Ability to work independently in community settings while maintaining consistent program standards, participant safety, confidentiality, timeliness, and accountability to grant deliverables.
- Strong written, verbal, organizational, and technology skills, including Microsoft Office Suite, case-management/reporting systems, electronic records, mobile devices, and secure handling of protected and personally identifiable information.
Skills
- Ability to respond appropriately when a participant presents suicide risk or behavioral-health crisis by immediately following established screening, escalation, emergency-response, and referral protocols.
- Occasional evening or weekend work may be required for outreach, community events, participant needs, or time-sensitive grant deliverables.
