Join The Care Team as a Hospice LPN and make a meaningful impact by providing compassionate, hands-on care and support to patients and families during lifes most important moments. We are seeking a dedicated Licensed Practical Nurse to deliver high-quality patient care, support the interdisciplinary team, and help ensure comfort, dignity, and exceptional end-of-life care in home and facility settings.
Schedule, Hours, & Territory
Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
Up to a 1-hour radius of Mishawaka, IN

Key Responsibilities
Provide nursing care as delegated by the RN
Monitor patient status and report changes
Support comfort and symptom management
Document care provided in the EMR
Collaborate with the interdisciplinary team
Qualifications
Active Licensed Practical Nurse (LPN) license
Hospice or long-term care experience preferred
Field Role Requirements
Active drivers license
Ability to pass a criminal background check
Ability to pass a Motor Vehicle Record (MVR) check
Current TB testing in accordance with agency policy
Current CPR certification as required for the role
