Responsibilities
- Provide direct patient care including administering medications, wound care, and monitoring vital signs.
- Collaborate with physicians, nurses, and other healthcare professionals to develop and implement patient care plans.
- Document patient information accurately and timely in compliance with company and regulatory policies.
- Educate patients and their families on health maintenance and disease prevention.
- Maintain a clean and safe environment for patients.
- Respond promptly to patient needs and emergencies.
Requirements
- Valid LPN license in the state of Michigan.
- Previous experience in home health care or a related nursing role is a plus.
- Excellent communication and interpersonal skills.
- Ability to work independently and effectively manage time and priorities.
- Basic computer skills for documentation purposes.
- Reliable transportation to travel to patient homes in Southfield, MI area.
