University Home Care Inc is seeking a dedicated Field OT to provide exceptional occupational therapy services to our clients in Oakland, MI. In this role, you will deliver personalized care in the field, helping individuals improve their daily living skills and achieve their personal goals. You will work collaboratively with a multidisciplinary team to develop and implement effective treatment plans tailored to each clients unique needs.
Responsibilities
- Perform comprehensive occupational therapy evaluations and assessments in clients' homes.
- Develop and implement individualized treatment plans to improve clients functional abilities.
- Provide hands-on therapeutic interventions to enhance clients independence and quality of life.
- Document all client interactions, progress notes, and treatment plans accurately and timely.
- Communicate regularly with clients, caregivers, and healthcare professionals to coordinate care.
- Maintain compliance with company policies, professional standards, and industry regulations.
Requirements
- Licensed Occupational Therapist (OT) in the state of Michigan.
- Proven experience providing occupational therapy services in a home care or field setting preferred.
- Strong interpersonal and communication skills.
- Experience with Kinnser
- Ability to work independently and manage a flexible schedule.
- Valid drivers license and reliable transportation.
- Commitment to delivering compassionate and patient-centered care.
