Palliavte Care LPN

University Home Care Inc is seeking a dedicated and compassionate Palliative Care LPN to join our team in Macomb, MI. This role involves providing high-quality nursing care to patients with serious illnesses, focusing on improving their quality of life through symptom management and emotional support.

Responsibilities

- Provide direct nursing care and symptom management to patients with life-limiting illnesses.

- Collaborate with physicians, interdisciplinary teams, patients, and family members to develop and implement individualized care plans.

- Monitor patient health status and report changes to appropriate healthcare providers.

- Educate patients and families on disease progression, treatment options, and comfort measures.

- Maintain accurate and timely patient records in compliance with regulatory standards.

- Support patients emotional and psychosocial needs in a compassionate manner.

- Ensure adherence to infection control and safety procedures.

Requirements

- Current Licensed Practical Nurse (LPN) license in the state of Michigan.

- Experience in palliative care, hospice, or related nursing field preferred.

- Strong communication and interpersonal skills.

- Be able to travel to Wayne & Oakland County

- Ability to work independently and as part of a multidisciplinary team.

- Compassionate, patient-centered approach to care.

- Basic computer proficiency for documentation and communication.
