WHAT YOU'LL DO
• Conduct patient assessments and develop individualized plans of care.
• Coordinate care with physicians, patients, families, and other healthcare professionals.
• Educate patients and caregivers on care plans, treatments, and ongoing health needs.
• Supervise and support LPNs, HHAs, and CNAs to promote quality patient care.
• Review and update plans of care as needed.
• Maintain accurate, timely, and complete clinical documentation.
• Participate in case conferences and quality improvement initiatives.
• Help ensure patients receive safe, compassionate, and effective care in their homes.

WHAT WE'RE LOOKING FOR
• Graduate of an accredited nursing program.
• Active Indiana RN license or a qualifying multistate license recognized by Indiana.
• Minimum of 2 years of nursing experience; home health experience preferred.
• Current CPR certification.
• Strong clinical, communication, leadership, and organizational skills.
• A caring, patient-centered approach and a genuine desire to make a difference.
