- Develop and implement individualized treatment plans with meaningful, measurable goals focused on mobility, safety, and independence.

- Provide individual and small-group therapy, including therapeutic exercises, gait training, balance activities, transfer training, and functional mobility interventions.

- Help participants improve physical functioning, reduce the risk of injury, and develop strategies to navigate everyday activities safely.

- Assess and recommend mobility aids, adaptive equipment, durable medical equipment, and environmental modifications.

- Monitor participant progress, reassess functional abilities, and adjust treatment plans to support continued improvement.

- Educate participants, families, caregivers, and staff on safe mobility techniques, positioning, equipment use, and strategies that support long-term success.

- Collaborate with Occupational Therapists, nursing staff, service coordinators, and other interdisciplinary professionals to provide coordinated, participant-centered care.

- Provide clinical direction and supervision to Physical Therapist Assistants (PTAs), as applicable.

- Maintain accurate and timely evaluations, progress notes, treatment documentation, and required billing records.

What We're Looking For

- Current Massachusetts Physical Therapist license required.

- Degree in Physical Therapy from an accredited program.

- Current CPR/First Aid certification.

- Experience working with individuals with acquired brain injuries, neurological conditions, or in community-based rehabilitation settings preferred.

- Strong clinical assessment, treatment planning, and problem-solving skills.

- Excellent communication and interpersonal skills, with the ability to work effectively with individuals with diverse physical, cognitive, and communication needs.

- Ability to work collaboratively within an interdisciplinary team.

- A compassionate, creative, and patient-centered approach to rehabilitation.
