Position Summary
The Registered Nurse (RN) is responsible for providing skilled nursing care, conducting patient assessments, developing care plans, and coordinating services to ensure optimal patient outcomes. The ideal candidate is compassionate, reliable, and passionate about improving the quality of life for patients receiving home healthcare services.
Essential Responsibilities
- Perform comprehensive patient assessments and ongoing evaluations.
- Develop, implement, and update individualized care plans.
- Administer medications and treatments as prescribed by physicians.
- Monitor patient progress and document findings accurately and timely.
- Educate patients and family members regarding health conditions, treatments, and disease management.
- Coordinate care with physicians, therapists, caregivers, and other healthcare professionals.
- Maintain compliance with state, federal, and company regulations.
- Participate in case conferences and interdisciplinary team meetings.
- Respond appropriately to changes in patient conditions.
Qualifications
- Current and unrestricted Tennessee Registered Nurse (RN) license.
- Graduate of an accredited nursing program.
- Minimum of one year of nursing experience preferred.
- Home health experience preferred but not required.
- Strong assessment, clinical, and critical-thinking skills.
- Excellent communication and interpersonal abilities.
- Reliable transportation, valid driver's license, and auto insurance.
- Ability to work independently and manage time effectively.
