Responsibilities
- Provide skilled nursing care to patients in their homes, including assessment, treatment, and medication administration.
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
- Monitor patient progress and adjust care plans as necessary to ensure optimal health outcomes.
- Educate patients and their families on health conditions, treatments, and self-care techniques.
- Document all patient interactions, treatments, and outcomes accurately and timely according to company standards.
- Coordinate with physicians and other healthcare professionals to provide comprehensive patient care.
- Maintain patient confidentiality and adhere to all regulatory and compliance standards.
Requirements
- Current Registered Nurse (RN) license in the state of Michigan.
- Previous experience in home health nursing visits or related field preferred.
- Excellent clinical assessment, communication, and organizational skills.
- Ability to work independently and manage a flexible schedule.
- Reliable transportation to travel to patients homes within Livonia, MI.
