About this role

University Home Care Inc is seeking a compassionate and dedicated Hospice Registered Nurse to join our team. This role involves providing expert nursing care and support to patients and their families during end-of-life care, ensuring comfort and dignity through every stage of the hospice process.

Responsibilities

- Provide skilled nursing care and assessment to hospice patients in their homes.

- Develop and implement individualized care plans in collaboration with the interdisciplinary team.

- Administer medications and treatments as prescribed by physicians.

- Educate patients and family members on care techniques, symptom management, and disease progression.

- Document all nursing care and patient observations accurately and thoroughly.

- Participate in regular team meetings and professional development sessions.

- Advocate for patient needs and coordinate care with other health professionals and hospice staff.

Requirements

- Current and valid Registered Nurse (RN) license in the state of Michigan.

- Minimum of 1 year nursing experience, preferably in hospice, palliative care, or home health.

- Strong communication and interpersonal skills.

- Ability to work independently and make sound clinical decisions.

- Compassionate and patient-centered approach to care.

- Reliable transportation and valid drivers license for home visits.
