SHIFT: Saturday and Sunday - 7AM - 7PM
FLSA Status: NON-EXEMPT – This position is classified as Non-Exempt under the Fair Labor Standards Act and is eligible for overtime pay.
Job Summary: As a key member of our multidisciplinary team, you will manage incoming calls and referrals from individuals in crisis, concerned community members, and partner organizations- ensuring timely triage, placement, and care coordination. Youll use your knowledge, judgment, and collaborative spirit to support those navigating behavioral health challenges, working closely with internal teams and external providers to provide appropriate services and resources.
Essential Functions:
- Is the point of contact for the incoming calls and assists with identifying and connecting client with immediate and ongoing needs; maintains working knowledge of community resources and acts as a broker between clients, the community and caregivers.
- Works as part of a multidisciplinary team to provide placement, referral, and admission, through phone and referring agencies.
- Manages Diversus Health Emergency Response communications.
- Facilitates ongoing communication with clients and referring providers to update placement information and communicates with client family members to provide updates on client care.
- Coordinates with clinical team to provide care to clients in crisis in the community, in coordination with outpatient care, and 988 hotline.
- Receives and reviews all incoming referrals for ATU level of care.
- Accepts appropriate referrals for ATU placement based on established criteria and coordinates with referring agencies for nursing reports and transportation to ATU.
- Refers clients at the crisis walk-in center for internal and external placement.
- Obtains initial insurance pre-authorizations for accepted clients.
- Maintains accurate documentation of all client interactions in the electronic health record system.
- Supports continuity of care and promotes safety and stability in crisis situations.
- Help with ongoing continuity of care through follow-up calls.
Qualifications
Knowledge/Skills/Abilities:
- Bachelors Degree in a behavioral health or healthcare related field or equivalent experience.
- One year of volunteer or professional experience in the behavioral health field, preferred
- Desire to work with people who have mental illness
- A thorough understanding of systems of care and integration of physical and behavioral health
- Adapts to differences in organizational cultures (primary care, substance use, social services, etc.)
- Ability to take initiative and use critical thinking to solve problems independently
- Excellent verbal and written communication skills and can easily build rapport
- Proficient in Microsoft word and Excel and can type at least 25+ w.p.m.
