Qualifications

Job Summary:

The Licensed Practical Nurse (LPN) provides nursing care to PACE participants in the home, center, and clinic in accordance with the participants Plan of Care, provider orders, organizational policies, and LPN scope of practice. The LPN works closely with the RN Care Coordinator (RNCC) and interdisciplinary team (IDT) to support safe, coordinated, participant-centered care. This position provides in-home nursing visits, clinical and home care support, participant/family education, and participates in the organizations on-call rotation.

Duties/Responsibilities:

- Provide nursing services and interventions as outlined in the participants Plan of Care and/or provider orders.

- Provide scheduled and PRN nursing visits in participants homes and provide nursing support in the center and clinic as needed.

- Monitor participant condition, identify changes or concerns, and promptly communicate findings to the RNCC and appropriate IDT members.

- Support the RNCC and IDT with nursing follow-up, care coordination, and implementation of participant care plans.

- Administer medications and treatments as ordered and provide participant and caregiver education.

- Collaborate with home care, clinic, nursing, and other clinical staff to support continuity of care.

- Assist with completing home care assessments and recommendations.

- Participate in the on-call rotation and respond to participant/caregiver needs in accordance with established on-call procedures and scope of practice.

- Complete accurate and timely documentation of visits, participant contacts, interventions, and follow-up.

- Participate in IDT meetings, staff training, competency activities, quality improvement initiatives, and other assigned activities.

- Maintain professional licensure and required competencies.

- Perform other duties and responsibilities as assigned.

Education, Licenses, Certifications and Experience

- Current Oklahoma LPN License without restrictions

- Either one year working with the frail and elderly population or approved training.

- Current CPR certification

Physical Requirements:

- Ability to speak, hear, and read English.

- Lift up to 100lbs. from floor to 19 and from 19 to 25 (occasional)

- Walk up to .25 miles (frequent)

- Sustained forward bending in sitting (occasional)

- Push/Pull up to 100 lbs of force at 36 (occasional)

- Repetitive squat motion (occasional)
