The OSU General Internal Medicine (GIM) Complex Care Program seeks a highly motivated and patient-centered Community Outreach Analyst to advance community engagement, referral development, patient access, and program operations. This position serves as a key liaison among patients, caregivers, referring providers, healthcare organizations, community partners, and the interdisciplinary Complex Care team, working to strengthen awareness of program services and improve access to care for individuals with complex healthcare needs.

The Community Outreach Analyst develops and coordinates community engagement, educational, and outreach initiatives designed to increase awareness of the Complex Care Program, strengthen referral pathways, and expand access to care throughout Ohio. The position cultivates relationships with healthcare providers, hospitals, advocacy organizations, community agencies, and institutional partners, serving as a resource and connector to promote collaboration, support program growth, and assist patients and families in navigating complex healthcare systems.

In addition to outreach and relationship-building activities, the Community Outreach Analyst supports referral and intake processes, verifies program eligibility, facilitates patient access and scheduling, and assists individuals with complex medical, developmental, congenital, and pediatric-onset conditions as they transition into adult healthcare services. The position collaborates closely with physicians, nursing, social work, dietitians, administrative staff, and external providers to ensure seamless care coordination, efficient program operations, and a high-quality patient experience.
