Independence Home Health LLC is seeking a compassionate, dependable, and highly flexible Transitional Aide to join our team!

PAY & HOURS
• Pay: $18.00 per hour
• Guaranteed 40 hours per week

REQUIREMENTS
• Exceptional scheduling flexibility is required. This position requires availability around the clock (24/7) to assist with unexpected staff absences, new client needs, and short-notice scheduling changes. Availability may include days, evenings, weekends, and occasional overnights.
• Reliable transportation.
• Ability to assist clients during transitional periods of care.
• Strong communication and time-management skills.
• Dependable, professional, and compassionate demeanor.
• Ability to follow care plans, agency policies, and instructions from supervisors.

KEY RESPONSIBILITIES
• Must be willing and able to travel within approximately a 1-hour driving radius to meet client needs and cover shifts.
• Reliable transportation and a valid driver's license
• Provide support and assistance to clients during transitions in care.
• Assist with activities of daily living as needed and within the scope of the role.
• Follow established care plans and agency policies and procedures.
• Communicate effectively with supervisors, caregivers, and other members of the care team.
• Help promote client comfort, safety, and independence.
• Respond to changing client needs and scheduling requirements.
