Responsibilities
- Provide direct nursing care to patients in a home care setting.
- Administer medications and treatments as prescribed by physicians.
- Monitor patient health and report any changes or concerns to the healthcare team.
- Maintain accurate and timely documentation of patient care and progress.
- Collaborate with patients, families, and healthcare professionals to develop and implement care plans.
- Ensure adherence to all company policies, procedures, and state nursing regulations.
Requirements
- Valid Licensed Practical Nurse (LPN) license in the state of Michigan.
- Previous experience in home care or a related healthcare setting preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate, patient-focused attitude with a commitment to quality care.
